The Advocates for Responsible Care (ARxC) is developing a new model for health insurance that meets the health care needs of Georgia citizens.

We want to know what you, as a citizen of Georgia, want from your health insurance plan. By taking the online ARxC Health Plan Survey© you can play an active role in deciding the future of health insurance in Georgia.

Please go to www.surveymonkey.com/s/NQ88GSM and complete the survey. You must be at least 18 years of age and a citizen of Georgia to participate. The survey may take between 15 to 20 minutes to complete. Your participation is entirely voluntary; there are no risks or immediate benefits to taking the survey. You will not be linked to your responses, and the survey has been approved by the Emory University Institutional Review Board to guarantee that your rights as a participant are upheld.
